In-Home Services
In-Home Services provides assistance to Forsyth County residents, 18 years of age and older, who are physically and/or mentally disabled and need
services that we provide in order to remain in their home.
Social Workers assess and evaluate each adult to develop and implement a service plan to meet the needs. This would include locating and contacting
other providers; coordinating and monitoring the delivery of services.
