Temporary Economic Assistance and Maintenance

Family and Children's Medicaid

Family & Children's MedicaidThe Family and Children's Medicaid program pays medical bills for eligible low-income families and children. Each month a Medicaid identification card is received which is presented when services are needed. The selection of a primary care provider is generally required for families and children receiving most categories of Medicaid.

You may apply for Family and Children's Medicaid Monday through Friday from 8:00 a.m. to 4:30 p.m. at:
    741 North Highland Avenue
    Winston-Salem, NC 27101

Please bring the following pieces of information for all family members with you:

  • Birth Certificates
  • Social Security Numbers
  • Verification of income of any kind
  • Verification of assets such as bank accounts, insurance policies, property, etc.
  • Verification of any medical expenses for the past three (3) months

You may also apply for Family and Children's Medicaid and NC Health Choice by completing a mail-in Medicaid/NCHC application. To obtain a copy of the mail in application in English, click here. For a copy of the mail in application in Spanish, click here.

The completed mail-in application must be returned to the Dept. of Social Services Building at 741 North Highland Avenue or mailed to:

For families leaving Work First cash assistance eligibility can continue for Family and Children's Medicaid for 12 months after they no longer receive a monthly TANF check.
