Healthy Start / Baby Love Plus
This case management program is for post-partum women and helps improve their interconceptional
health, maximize positive future birth outcomes, and address racial disparities
in infant mortality. This program is only available to women who were enrolled in
the Baby Love Maternity Care Coordination Program and have
identified risk factors that may negatively impact their health and future birth
outcomes. A Public Health Social Worker provides home visits, support, education,
and advocacy while empowering women to develop healthy life skills and make informed
