Well Child Clinic

This clinic is designed to meet the health care needs of children that are not served by another health care provider and that are uninsured/underinsured. (The clinic does not perform sports physicals, college physicals or camp physicals).

Children are seen in clinic B at the Forsyth County Public Health Department located at 799 N. Highland Avenue. The clinic accepts appointments for children ages 0-21 years for well child visits. Visits include:

  • complete nursing physicals
  • nursing assessments
  • day care and kindergarten physicals
  • immunizations and lead screening tests
  • developmental screenings

To schedule an appointment, please call (336) 703-3268, Monday-Friday from 8:00 am until 5:00 pm. Appointments are scheduled for Wednesdays between 8:30 am and 5:00 pm. Effective January 2011, the new Well Child Clinic operational day will change from Wednesday to Thursday. You will need to bring your child’s immunization record and social security card to the appointment.

In the event of inclement weather clinics will operate on a walk-in basis (first come, first serve)
